Agreement
Dr Clean provides for payment of a recruitment incentive to an individual who is newly appointed to a position in the Dr Clean. This incentive must be authorized before the employee enters on duty in the position for which the employee was recruited. The employee must sign this service agreement before the recruitment incentive may be paid. The Recommending official completes this form and obtains the employee’s signature on the last page of the form
